Documents required for the application
- 1. Form TM.7 : one 4 x 6 cm photograph, and a fee of 1,900 Baht (foreigners must apply in person).
- 2. Copy of passport : (including photo page, visa, latest entry stamp, departure card (TM.6), re-entry permit, and last extension of stay, with each page signed by the foreigner).
- 3. Employment certification : from the Immigration Office (Form STTM.1).
- 4. Copy of work permit : (all pages with information, signed by the foreigner).
- 5. Copy of legal entity registration documents : such as partnership certification, not older than 6 months.
- 6. Copy of the shareholder list : not older than 6 months.
- 7. Copy of balance sheet : profit and loss statement, tax return (Por Ngor Dor 50), receipt, and SBCh.3 (endorsed by the Revenue Department, Ministry of Commerce, auditor, or original documents).
- 8. Copy of Por Ngor Dor 1 : showing names of Thai and foreign employees for the past 3 months, with receipt.
- 9. Latest Copy of Por Ngor Dor 91 : of the foreign applicant, with receipt (if available).
- 10. Copy of Social Security payment submission : (SPS 1-10) for the past 3 months, with receipt.
- 11. Copy of Por Por 30 or Por Por 36 : for the latest 3 months, with receipt.
- 12. Documentation justifying the need to hire foreign staff : such as advertisements for Thai applicants.
- 13. Map showing the location of employment : stamped by the company and signed by an authorized person.
- 14. Other documents : as specified by the Immigration Commission (Form STTM.2).
- 15. Photos of the business premises : stamped by the company, showing Thai and foreign employees working.
- 16. Original documents : for items 5, 6, 7, 8, 9, and 10 to be presented.
- 17. Evidence of family relationships : if applicable.

- 5. Copy of teaching license : or certification/exemption from the Teachers' Council (except for higher education).
